Form Assistant features the Electronic Proposal Approval Record (ePAR) application.

LOGIN INSTRUCTIONS:

• If you have not been issued a Form Assistant login, please click on the "New User" button above and follow the registration instructions on the "New User" page.
• If you have previously logged into the Form Assistant or have just completed the registration process:
1. Select "Epar - Electronic Proposal Approval Record" by ("clicking") once on that phrase;
2. Enter your USC Employee ID used for the registration process and Form Assistant password;
3. Click "Login to Form Assistant" button above.
• If you previously logged into the Form Assistant but have forgotten your password:
1. Enter your USC Employee ID;
2. Click "Forgot Password" button above and follow the instructions that will appear.

GENERAL USAGE NOTES:

• Please use the "tab" key when moving from one field to another. Pressing the "enter" key while using Internet Explorer will send you to another page of the application.

NOTICE: Use of this and all other USC Business Processes is for Official USC Purposes Only. Violators will be Prosecuted!

All Activity is Logged and Logs Are Inspected and Archived.
